Pigmented lesions are an accumulation of melanin cells that produce a brown spot on the surface of the skin. These spots are often referred to as age spots, freckles, melasma, or solar lentigo and they are the result of sun exposure over a period of time. These lesions can be permanently removed by laser (intense pulsed light) therapy.

Other pigmented lesions include moles or nevi, which occur spontaneously. They are not the result of sun damage and therefore, should be evaluated before any laser treatment. It is very important to have an accurate diagnosis of any suspicious-looking moles prior to treatment, especially when hair removal from a mole is desired. Similarly, birthmarks are hereditary and should not be treated without proper assessment by a dermatologist.

The mechanism of pigmented lesion removal is based upon the selective destruction of melanocytes and melanosome-containing epidermal keratinocytes by the energy of the laser light. Their destruction is associated with some crusting and epidermal damage, which resolves within a one to two week period.Healthy skin is protected from thermal damage by the application of ice packs, cooling gel, and cyrogenic spray to the laser handpiece. Topical anaesthetics are not recommended during the procedure since the procedure is virtually painless and non-invasive.Immediately after treatment, the brown areas or spots may turn darker in colour over the next day. A mild sunburn-like sensation is anticipated which disappears within 2–24 hours. The area surrounding the lesion may turn red and slightly puffy. These are all expected outcomes and will resolve 2–3 days. With proper post-treatment care, there is no down-time and the dark areas will slough off in 7–10 days, leaving beautiful, blemish-free skin.
